Apply for a Liquor License in Lexington

To apply for a liquor license, complete the entire form below as it pertains to your business and business needs. 

Print the form and submit to City Hall with the following documents: 

  • Certificate of Insurance

  • Surety bond

  • Copy of current State Liquor License

  • Photo ID's for business owner(s) and/or names of those listed on liquor license application. 

  • Payment for license + $75 per gaming terminal
